Registered Nurse Case Manager | Melrose Park, IL

The rn case manager provides health services and care coordination for youth in residential and school settings. Responsibilities include managing medications, performing physical assessments, and collaborating with providers and families to ensure optimal care.

Candidates must be a graduate of an accredited nursing program with a current, unencumbered rn license in illinois and at least one year of experience. Previous experience with pediatric populations or individuals with developmental disabilities is preferred.
