- Community Health Educator GENERAL DESCRIPTION The Community Health Educator advances the mission of Broadlawns Medical Center (BMC) by delivering education, outreach, and engagement to patients, employees, students, and community members.
- This role promotes awareness of BMC services, improves access to high-quality care, and supports initiatives that reduce barriers to healthcare.
- Under the direction of the Director of Access and Engagement, the Community Health Educator serves as a liaison between BMC and community stakeholders.
- This position helps individuals navigate healthcare systems and connects them with appropriate resources to support positive health outcomes.
- Roles & responsibilities 1.
- Develops and delivers educational programs for patients, families, employees, and community members that address the social drivers of health and support BMC’s mission, services, and key initiatives 2.
- Supports the Director of Access and Engagement in building and maintaining partnerships with local schools and educational organizations.
- 3.
- Educates high school and college students about healthcare career pathways, including opportunities within BMC.
- 4.
- Provides community-based education to help organizations, stakeholders, and partners better understand hospital services and how to navigate them effectively.
- 5.
- Assists with onboarding, orientation, and ongoing support of BMC volunteers to ensure alignment with organizational needs.
- 6.
- Participates in community outreach initiatives that promote healthy lifestyles and address barriers to healthcare access, specifically, the social determinants of health.
- 7.
- Collaborates with internal teams, including Community Health Navigation, Case Management, Financial Counseling, and Patient Advocacy, to identify and reduce patient barriers.
- 8.
- Supports departmental initiatives that enhance community engagement and improve patient experience.
- 9.
- Provides administrative and logistical support to the Access and Engagement, Volunteer Services, and Language Services teams.
- 10.
- Assists with data collection, tracking, and reporting of program outcomes and impact.
- 11.
- Coordinates events, educational materials, and communications related to outreach and engagement activities.
- 12.
- Performs other duties as assigned.
- Minimum qualifications 1.
- Bachelor’s degree in a related field, or equivalent combination of education and four years of relevant experience.
- 2.
- Minimum of two years of experience developing and delivering educational programs.
- 3.
- Experience working with populations facing communication, cultural, or access barriers.
- 4.
- Experience navigating healthcare and social service systems.
- Preferred qualifications 1.
- Healthcare setting experience.
