Job description
Description The Family Support Specialist supports the delivery of family-centered services at PMCH by coordinating daily activities, engaging with the community, and maintaining clear communication with colleagues and participants. This role contributes to program goals, tracks outcomes, and supports continuous improvement.
Essential Functions Outreach families referred through Connecting NJ or a Service Provider Conduct family-centered and strength-based Home Visits based on the assigned enrolled family’s level of service Conduct the FROG Scale (parent survey tool) with parents to identify strengths/risk factors to support the family throughout home visitation services Utilize Parents As Teachers curriculum to help improve parent-child interaction Assist in strengthening the parent-child relationship Assist parents in improving their skills to optimize the home environment Assist in increasing the family support system and increase the family’s ability to problem solve and assume the role of advocating for themselves and their children Assist the family in establishing goals and a plan for the accomplishment of those goals. Requirements
Bachelors’ degree preferred Minimum 2 years of experience in working with children (0-3 years of age) and families Bilingual required: English and Spanish/other Excellent verbal, written and interpersonal communication skills Experience working with community agencies, including implementing referral processes Ability to relate to families from a strength-based model even in an apparently chaotic family environment; ability to approach families from a family-centered service model Flexible availability for home visits Flexible availability for community outreach/tabling events Ability to travel throughout New Jersey to attend required core trainings, curricula training and ongoing trainings required by the model and/or agency Infant mental health endorsement level I or II preferred DEIB Drivers for Success PMCH aims to foster a workplace culture that prioritizes diversity, equity, inclusion, and belonging.
