Provides vocational support to individuals in community and facility settings to help them achieve goals in their Life Plan. Acts as a liaison between clients and employers while monitoring work quality and documenting progress.
Requires a High School diploma or GED and at least six months of employment experience. Candidates must possess good math and communication skills and be a qualified New York State driver.
Overview Provides supports to the people we serve in community and facility based vocational activities.
Primary duties and responsibilities include but not limited to: Supports the development and ongoing revisions to individual’s Staff Action Service Plan by providing input and information. Tracks individual’s progress towards desired goals and objectives in Life Plan. Segments job duties and assignments into specific steps to facilitate the learning of the people we serve. Monitors individual’s level of performance including determining when support can be maintained, increased, or gradually faded. Monitors the quality of the work that the people we serve produce, in order to ensure it adheres to industry standards and all tasks are completed within an acceptable time frame. While in assigned community worksites, represents agency in a professional and exemplary manner. Serves as a liaison between people we serve and worksite employees/employers to facilitate appropriate work readiness skills training and professional work relationships. Maintains accurate records on job coaching services provided to each person we serve. Documents and recommends to supervisor any action necessary to overcome employment barriers. Responds to crisis situations in a humane and proactive manner and in accordance to policy procedures and training. Develops and maintains written documentation, as requested. Makes all decisions based on agency vision and values. Performs additional related duties as needed and/or as requested by supervisor and/or administration.
