Community Health Advocate Community Health Advocate

Position Summary

The Community Health Advocate (CHA) supports community outreach, education, and relationship-building efforts that advance the hospice and serious illness care mission. Working under the direction of the Community Engagement Manager, the CHA assists with YMCA Resource Center activities, community education, and resource navigation, helping individuals and families access supportive services. Through relationship-based engagement, workshops, and community partnerships, the CHA addresses questions, reduces misconceptions, gathers community feedback, and strengthens awareness, access, and trust in hospice, palliative care, and related supportive resources. Community Health Advocate Position Qualifications: Health-related certification, license, or equivalent experience in community health, healthcare outreach, or related field; bachelor’s degree in public health, social work, health education, or related field preferred. Strong communication skills with the ability to explain healthcare services clearly to diverse audiences. Demonstrated ability to build trust and relationships within the community. Ability to work effectively with individuals from diverse cultural and socio-economic backgrounds. Strong organizational, time-management, and problem-solving skills. Basic computer proficiency, including mobile devices and Microsoft Office applications. Alignment with Hosparus Health’s mission, values, and employee health requirements. Valid Driver’s License Must have flexibility to work evening and weekend events.
