- The base pay range for this position is $39.55-$58.55 Hourly however, base pay offered may vary depending on job-related knowledge, skills, and experience.

- Overview: The Certified Diabetes Care and Education Specialist (CDCES) provides comprehensive diabetes self-management education and support (DSMES) across a network of outpatient clinics including Endocrinology. This role partners with interdisciplinary care teams to improve clinical outcomes, enhance patient self-management, and reduce complications associated with diabetes and related conditions. The CDCES delivers patient-centered care using evidence-based guidelines and behavior change strategies.

- Life: Provide individualized and group diabetes self-management education and support aligned with standards from the American Diabetes Association and Association of Diabetes Care & Education Specialists Assess patient needs, readiness to change, health literacy, and barriers to care Support newly diagnosed patients and those with uncontrolled diabetes or complex regimens in collaboration with primary care and endocrinology Develop and implement personalized care and education plans Educate patients on medication management (including insulin), blood glucose monitoring, nutrition, physical activity, and risk reduction Train patients on diabetes technology, including continuous glucose monitoring (CGM) and insulin delivery systems Collaborate with physicians, advanced practice providers, pharmacists, dietitians, and nurse care managers across clinic sites Monitor patient progress and outcomes; adjust care plans accordingly Document all patient interactions in the electronic health record (EHR) in compliance with regulatory and billing requirements Support population health initiatives within the clinic network Participate in quality improvement projects and help track program performance metrics (e.g., A1C reduction, adherence, hospitalization rates)

- Have: Current certification as a Certified Diabetes Care and Education Specialist (CDCES) Active, unrestricted clinical license in a qualifying discipline (e.g., RN, RD/RDN, PharmD/RPh, NP, PA) Bachelor’s degree Minimum of 2–3 years of experience in diabetes care, education, or chronic disease management Strong knowledge of current diabetes clinical guidelines and standards of care Experience with diabetes technologies (CGM, insulin pumps, smart pens)
