  • Shift: DayMake a Difference Every Day The Life and Wellness Coach supports individuals in identifying personal goals and building the skills needed to live more independently and confidently. This role provides mentorship, skill-based instruction, and positive behavioral support in both community and home settings. The position also assists the Employment Services Department through job coaching and workplace skill development. Responsibilities Support individuals in creating and reaching personalized plans for success in social, wellness, Teach skills related to independent living, communication, budgeting, self-advocacy, health, and workplace success Use positive behavioral strategies to encourage growth and confidence Work in a variety of settings, including homes, workplaces, and community spaces Lead groups, community activities, and wellness-based programs Develop community opportunities that promote health, safety, and inclusion Provide job coaching and workplace support to help individuals maintain employment Communicate with families, caregivers, and providers to ensure consistent support Build positive and trusting relationships with individuals served This position is full-time 37.5 hours a week, Monday-Friday (Schedule subject to change based on participant activities). Additional hours if needed to support the individual, must be approved by your supervisor. Why Join Our Team?   • QualificationsQualification/Education/Experience: 1. Required High School diploma, Associates, or Bachelors. 2. Required one years of full-time experience providing supported services to people with disabilities, one year of experience working with people with disabilities, and a willingness to engage in direct community services. 3. Commitment to high quality customer services. 4. Strong interpersonal, organizational, analytical, written and oral communication skills. 5. Great time management, attentiveness, problem solving abilities, reliable, adaptable, attention to details, effective decision making and prompt response time to deadlines/ documentation 6. Team player and proactive mentality 7. Valid driver’s license required 8.Bilingual preferred.
