Build relationships with clients and families, ensuring services reflect each person’s unique needs and wishes. Coordinate service agreements, authorizations, and referrals. Develop and implement strategies in collaboration with families and support teams. Maintain accurate electronic records (Quantum) and case notes. Provide advocacy and support during crises. Partner with community resources and internal teams to ensure holistic care. Travel throughout Cook County for client visits (about 70% in the community). What We’re Looking For:
