Our mission is to provide critical life assistance and professional mental health services with an emphasis on economically disadvantaged, marginalized youth, and senior adults in the greater South Florida metropolitan area. Summary
The Life Coach/Case Manager will have extensive experience working with lesbian, gay, bisexual, transgender, gender variant youth and their families. This position will assist young people transition into adulthood and connect to services in the area of housing, education, health, life-skills, relationships and more. Knowledge of community resources and their qualifications is critical.
Key Responsibilities: · One-on-one life-coaching/case management with LGBTQ Youth & Families
· Timely and through assessments, case planning and ongoing updates quarterly and as clients progress
· Timely and accurate documentation of case notes
· Linkage to services such as therapy, housing, education, life-skills groups and more
· Regular in-vivo teaching which may include riding the bus with youth, grocery shopping, visiting college campuses, viewing apartments, teaching youth how to cook and more
· Participation in regular case conferences with therapist as well as case-managers/peer support specialists
· Develop, implement & facilitate curriculum for Life Skills group as well as coordinate guest trainers/speakers
· Other duties as assigned
Education
Bachelor's Degree
Experience/Requirements: · One year of office experience required
· Strong organizational skills required
· Group facilitation skills required
· Strong communication skills (written and verbal)
· Experience with Microsoft Office Suite, Word, Excel, Outlook, Google Docs
· Able to handle many varied tasks at one time
· Able to prioritize tasks and self-motivate
· Experience working with diverse populations, various backgrounds and cultures
· Must have the ability to work independently and within small groups
