This role is responsible for the development, coordination and implementation of specialized program services, providing program oversite and consultation on specific grant programs while provide training, support, and enrichment to assigned clients/families.

Minimum Qualifications

EDUCATION, CERTIFICATION, AND/OR LICENSURE: 1. Bachelor's degree in a human service-related field such as psychology, sociology, social work, counseling, special education. 2. Valid WV driver’s license, valid vehicle insurance and a reliable vehicle. 3. State criminal background check and Federal (if applicable), as required for regulated areas. EXPERIENCE: 1. Completion of a practicum/internship during the 4 year degree training in the human services field Completion of a practicum/internship during the 4 year degree training in the human services field OR one year of on the job supervision at Healthy Minds.

Preferred Qualifications

EDUCATION, CERTIFICATION, AND/OR LICENSURE: 1. Master's degree (BA/BS/MA/MS) degree in a human service related field such as psychology, sociology, social work, counseling, special education, etc. EXPERIENCE: 1. Two years of post degree experience CORE DUTIES AND RESPONSIBILITIES: The statements described here are intended to describe the general nature of work being performed by people assigned to this position. Develop, coordinator, deliver/implement programs specific to grant population and disability groups and family members. 2. Coordinate services for grant recipients according to SOW, disability group and treatment plans. 3. Coordinate programs with necessary staff supports to complete SOW. 4. Develop, coordinate, deliver/implement specialty programs including all activities specific to SOW implementation. 5. Participate in and complete trainings necessary to deliver service programs in support of the grant SOW. 6. Train other program staff for cross training and to implement/assist in delivery of program SOW. 7. Implements appropriate interventions and documents patient’s responses in the medical record (as appropriate) using identified formats. 8. Identifies thought patterns and behaviors associated with different types of psychiatric diagnoses and developmental ages and intervenes appropriately utilizing the master treatment plan. 9. Transports and supervises patients to appointments and activities to implement grant SOW. 10. Answers the telephone and responds to all calls, including Helpline calls, in an appropriate and professional manner. 11. Conducts self in a professional manner (communication and appearance) with recipients, families, and other associates to include maintaining all the confidential aspects of the program. 12. Completes and maintains the quantity and quality of documentation to meet data requirements, regulatory and organizations requirements. 13. Create and grow collaborative relationships with other service providers in the community.
