The Occupational Therapist will complete functional assessments and create individualized treatment plans while providing therapeutic interventions focused on activities of daily living, mobility, and safety. They will also communicate progress and collaborate with the interdisciplinary team and families.
Candidates must have a degree from an accredited Occupational Therapy program and an active Oregon OT license or eligibility. CPR certification is required, and experience in SNF, post-acute, or long-term care is preferred but not mandatory.

What You'll Be Doing: · Complete functional assessments and create individualized treatment plans
· Provide therapeutic interventions focused on ADLs, mobility, and safety
· Communicate progress and recommendations with the interdisciplinary team and families
· Collaborate with COTAs and Rehab Aides to ensure consistent, high-quality care

Requirements: What We’re Looking For:
· Degree from an accredited Occupational Therapy program
· Active Oregon OT license (or eligibility)
· CPR certification (or ability to obtain)
· SNF, post-acute, or long-term care experience preferred—but not required
· Strong communication, clinical reasoning, and teamwork skills
