SUMMARY The Social Worker provides critical support to individuals and families navigating illness and disabilities. This role helps resolve personal, financial, and environmental challenges while working in accordance with ADAPT Community Network (ADAPT) policies, regulatory requirements, and standards of the profession. The Social Worker empowers families to understand and negotiate educational and clinical systems, advocating for their needs. Additionally, this position fosters collaboration with other disciplines to ensure a comprehensive continuum of care and support.
ESSENTIAL DUTIES AND RESPONSIBILITIES The duties and responsibilities of the Social Worker will include, but are not limited to the following:
Participates in the Adapt Intake Processes
Develops case notes, case summaries / social histories, etc. for each student on his/her caseload including appropriate recommendations.
Assists families in obtaining services such as housing and residential placement, food stamps, Medicaid, Assistive Technology services; respite services; family reimbursement services; camp, vacation referrals, etc.
Integrates therapeutic intervention techniques into school programs or home of students served through consultation with parent/guardian and staff at meetings, case conferences or through other informal contacts.
Participates in interdisciplinary team meetings and annual reviews for purposes of planning, implementation, and monitoring of IEP’s.
Provide emotional and behavioral support to students, particularly those with disabilities.
Lead the school’s efforts to ensure students receive necessary social-emotional resources or

Facilitates communication with family and/or caregivers concerning issues such as attendance, transportation, medical status, and clearance, etc.
Conducts parent trainings and monthly workshops for parents in conjunction with other multidisciplinary team members
Responding promptly to family concerns and providing guidance on available resources.
Documents status of all active cases.
Assists with other duties such as arrival and dismissal, feeding, bath rooming and classroom management as needed.
Keeps abreast of changes and research in the field of Social Work.
Assures confidentiality of all information regarding students and families receiving services.
Communicates with staff about goals, rights of persons receiving services, confidentiality, family interaction, etc.
Participates in activities pertaining to student transitioning/Turning-Five Process
Serves and participates as a member of interdisciplinary team meetings, annual reviews, parent meetings, Special Review Committee meetings, In-Services, etc. as required.
Participates in in-service training programs as required.
Provides supervision and training for Social Work interns and novice Social Workers.
Attends professional meetings and conferences.
Performs other related duties as requested.

Candidates must hold a Master's degree in Social Work and possess a New York State Licensed Master Social Worker (LMSW) license. A New York State Bilingual Spanish Extension is also required for this position.
