Collaborate with a multidisciplinary team, including other therapy disciplines, physicians, and nurses, to develop holistic treatment plans that address the diverse needs of each patient. Maintain comprehensive documentation that reflects the care provided and communicates changes in treatment plans to the entire healthcare team. Actively participate in care plan with team leads/supervisor to discuss patient progress, share insights, and refine treatment strategies collaboratively.
