The Wellness Coach coordinates patient care by integrating medical and behavioral health services and setting individualized self-care goals. They deliver health education sessions and manage case referrals to ensure all psychosocial and medical needs are met.

About This Role

  • Engagement: Accepts warm hand-offs from medical and behavioral health providers to ensure seamless integration of care; engages patients (adults and children) and families in setting and achieving self-care goals through individualized care planning and support.

  • Coaching: Delivers in-person and virtual health education sessions—both individual and group—using evidence-based curricula to support patients’ self-management and wellness.

  • Services: Provides case management, tracks patient progress, coordinates referrals to community resources, and monitors ongoing care plans to ensure medical, behavioral, and psychosocial needs are met.
  • Scheduling &
  • Access to
  • Services: Coordinates and schedules appointments across multiple service lines, including medical, dental, nutrition, and behavioral health; educates patients on available Evara Health services and facilitates access.
  • Reporting &
  • Committee
  • Participation: Collects and reports program data, presents patient case reviews, participates in CMHI Oversight Committee, and supports special projects and organizational compliance with HIPAA, HRSA, and other regulatory standards.

Education and Experience

  • Bachelor’s Degree in Social Work, Health or Human Services field, preferred
  • Minimum of three years’ experience with minimum qualifications listed above

Requirements

A Bachelor's degree in Social Work, Health, or Human Services is preferred. Candidates should have a minimum of three years of relevant experience and must pass a Level 2 background screening.
