Qualifications

  • New York State Licensed Physical Therapist
  • Masters Degree in Physical Therapy from a nationally accredited college or university.
  • Qualifications must meet Medicaid eligibility standards
  • Willingness to complete Personalized System of Instruction (PSI) training modules in a timely manner.
  • An ability and willingness to provide direct Physical therapy services to students with disabilities.
  • Sensitivity to the communication, academic and social needs of the students with developmental disabilities at all levels of independence.

Responsibilities

  • Direct treatment of a minimum 50 sessions per week at the School Age level.
  • Daily completion of session notes.
  • Daily CM 2/attendance card completion
  • Monthly completion of departmental daily note/CM2 review for PT department. Submission of completed daily notes and CM2 billing on the last day of the month to School Age Coordinator.
  • Monthly updates and monitoring of Physical Therapy attendance cards/databases.
  • PT Quarterly/Annual CM5 Progress Reports – coordinates completion and collection of all PT reports, goals, and regression summaries. Submits copies to classroom teachers and School Age Coordinator for distribution to parent/school district
  • PT IEP Direct Quarterly updates—ensures completion of all IEP Direct updates according to HCDS due dates or annual review meeting schedules.
  • Ensures all paperwork in compliance with all regulatory agencies.
  • Instructional Programming/curriculum development to address occupational therapy needs of School Age students.
  • Maintain and organize treatment gym and supplies.
  • Assist in researching and ordering new equipment and supplies.
  • Attend parent and staff meetings and in-services offered by Hawthorne Country Day.
  • Conduct PT evaluations for students when requested by School Age Coordinator and School District(s).
  • Timely completion of paperwork including, but not limited to, daily notes, quarterly progress notes with attendance codes, parent correspondence, IEP goal selection, CM 2 forms, department attendance sheets and any other required documentation stating progress or regression as requested by school district or School Age Coordinator. .
  • Complies with on NYSED, DOH and HCDS paperwork requirements and IEP Direct
  • Complies with documentation requirements (including physician recommendations/ prescriptions)
  • Complies with all NYSED, DOH requirements/audits
  • Ensuring Medicaid compliance
  • Attends required trainings to ensure compliance with current requirements by NYSED, DOH, Medicaid and HCDS
  • Prepares closed files for storage at the end of each school year and summer
  • Take necessary courses/workshops to maintain PT licensure.

General Responsibilities

  • Provide appropriate treatment to school age children as outlined in the IEP goals and directives.
  • Provide ongoing classroom communication with the teachers as well as the parents in regards to child’s progress and assist in home program implementation.
  • Provide a safe and nurturing environment for the children to thrive in.
  • Follow all of the staff rules and regulations as outlined in the Staff Hand Book. Dress should be neat and appropriate for the environment. Refer directly to the Department Head in reference to any questions or concerns.
  • Acts as PT department liaison with institutes of higher education, for whom we are providing clinical training
  • Arrives at work on time everyday.
  • Protects the health, safety and welfare or all students at all times.
  • Assumes other responsibilities as assigned.

Physical Responsibilities

· Sitting, standing, bending, lifting, and moving intermittently during work hours.

· Must be able to cope with the mental and emotional stress of the position.

· Must possess sight and hearing senses, or use prosthetics that will enable these senses to function adequately so that the requirements of the position can be fully met.

· Must function independently, have personal integrity, be flexible and able to work effectively with other personnel.

· Must be in good general health and demonstrate emotional stability.
