Responsible for promoting health education and conducting outreach to increase community utilization of health services. Duties include leading educational sessions, identifying outreach sites, and providing referrals for medical and social support.
A bachelor's degree in a health or social service field is preferred, or one year of related work experience. Candidates must possess a valid driver's license and insurance.

Reports To: As assigned by Program Director
This position is responsible for health education and outreach. This position requires local travel, setting/adhering to own schedule and independent decision-making regarding patient recruitment and provision of services. Provides education though educational sessions, group presentations, referrals and follow-up to increase awareness and utilization of health services for the community.
HOURS: 8 per Day / 5 Days per Week
