Coordinate and deliver health risk appraisals and biometric testing to improve patient self-management and overall well-being. Conduct one-on-one wellness coaching and develop health education programs for employer groups.
Requires a Bachelor's degree in Health Promotion or an Allied Health field and working knowledge of adult education and wellness concepts. Must obtain a Coach certificate (WellCoaches or equivalent) and BCLS certification within specified timeframes.
Job Duties Coordinate and deliver health risk appraisals Analyze data from health risk appraisals and biometric testing Conduct one on one consultations (wellness coaching) for health improvement Create, organize and conduct health education programs Collaborate with other health experts and employer groups Ability to utilize behavioral interviewing tactics Execute incentive program management for companies Follows Mercy's safety guidelines, carries out job-specific safety duties and responsibilities, and promptly reports any unsafe conditions, situations, incidents and injuries. Evaluate program metrics specific to employer groups Use of Microsoft office programs
Ability to communicate well orally and in writing; via phone, email and face to face; establish rapport with clients/patients. Self starter; able to initiate and direct own activities in an organized manner, and make decisions within the framework of the responsibility and authority of the job. Ability to teach group classes or conduct individual consultations. Ability to drive a car. Professional Experience Working knowledge of principles in adult education, health education and wellness concepts required. Education Bachelor's degree in health Promotion or an Allied Health Field, with course work specific to health promotion and population management required. Licensure, Certification, Registration Must currently be credentialed in one of the below or obtain within 18 months of hire: 1) Current Coach certificate through WellCoaches or equivalent organization 2) Completion of the ICC (Iowa Chronic Care Consortium) Clinical Health Coach Training Program/Performance Evaluation Also, must obtain Basic Cardiac Life Support (BCLS) within the first 30 days of employment.
