The Case Manager will provide comprehensive, client-centered case management and ongoing support to individuals and families. Responsibilities include assessing clients' needs, developing action plans, connecting clients with resources, and maintaining accurate documentation.
Candidates must have a bachelor's degree in Social Work, Education, or a related field, along with two years of case management experience. Proficiency with Microsoft Office and a valid driver's license are also required.
