Our Values R – Respect I – Innovation S – Stewardship E – Excellence School-Based Case Manager Summary The School-Based Case Manager provides comprehensive case management services to children and adolescents receiving behavioral health services within the school setting. This role partners with students, families, school personnel, healthcare providers, and community agencies to identify barriers to academic and social success while coordinating services that promote emotional well-being, school engagement, and overall wellness. The School-Based Case Manager develops collaborative, strengths-based service plans, connects students and families with community resources, advocates for student needs, and monitors progress toward established goals. This position requires strong organizational skills, independent judgment, and the ability to build effective relationships within a multidisciplinary team while supporting Primary Health Solutions' mission of delivering whole-person care. The School-Based Case Manager works under the direct supervision of the Program Manager and collaborates closely with school administrators, teachers, counselors, behavioral health providers, and other PHS leaders. A Day in the Life Conduct ongoing assessments to identify student strengths, needs, barriers, and progress toward established goals. Develop, implement, monitor, and update individualized service plans in collaboration with students, families, school staff, and behavioral health providers. Coordinate behavioral health, medical, educational, and community resources to ensure students receive appropriate services. Serve as a liaison between families, schools, healthcare providers, and community organizations to promote continuity of care. Assist families in accessing transportation, housing, food assistance, insurance benefits, financial resources, and other community supports as appropriate. Advocate for students by helping reduce barriers to attendance, academic performance, emotional wellness, and behavioral success. Provide psychoeducation to students and families regarding behavioral health conditions, coping skills, available resources, and wellness strategies. Support crisis response activities by coordinating interventions, referrals, and follow-up services. Participate in multidisciplinary team meetings, school conferences, and care coordination meetings. Maintain accurate, timely, and compliant documentation within the electronic health record. Complete required outcome measures, treatment plan updates, and clinical record reviews within established timelines. Demonstrate proficiency with agency technology systems and documentation platforms. Attend required supervision, meetings, trainings, and professional development. Support quality improvement initiatives and meet established productivity expectations. Perform other duties as assigned. Core Competencies Maintain reliable attendance and punctuality. Complete required training and continuing education. Adhere to professional, ethical, FERPA, and HIPAA standards. Participate in supervision and apply feedback. Maintain confidentiality of student, family, and agency information. Demonstrate sound judgment, organization, and time management. Provide culturally responsive, family-centered services. Collaborate effectively with school staff, families, and community partners. Utilize required technology and documentation systems proficiently. Complete assignments accurately and within established timelines. Support a safe, inclusive, and welcoming school environment. Comply with all organizational policies and reporting requirements.

Success Requirements Education/Experience Bachelor's degree in Social Work, Human Services, Counseling, Psychology, Sociology, or a related behavioral health field preferred. Experience working with children, adolescents, families, schools, behavioral health, case management, or community-based services required. Knowledge of trauma-informed care, child development, community resources, and care coordination preferred. Language Skills Ability to read, interpret, and prepare professional documents, communicate effectively with students, families, school personnel, and community partners, and present information clearly. Reasoning Ability Ability to exercise sound judgment, prioritize competing responsibilities, solve practical problems, and adapt to changing situations. Computer Skills Working knowledge of Microsoft Office and the ability to learn electronic health record and documentation systems. Certificates, Licenses, Registrations Valid Ohio driver's license and reliable transportation required.
