Provide comprehensive case management for children and adolescents to remove barriers to academic and social success. Coordinate behavioral health, medical, and community resources while maintaining compliant documentation in electronic health records.
A bachelor's degree in Social Work, Psychology, or a related behavioral health field is preferred. Experience working with children, adolescents, and families in a case management or school setting is required.
Success Requirements Education/Experience Bachelor's degree in Social Work, Human Services, Counseling, Psychology, Sociology, or a related behavioral health field preferred. Experience working with children, adolescents, families, schools, behavioral health, case management, or community-based services required. Knowledge of trauma-informed care, child development, community resources, and care coordination preferred. Language Skills Ability to read, interpret, and prepare professional documents, communicate effectively with students, families, school personnel, and community partners, and present information clearly. Reasoning Ability Ability to exercise sound judgment, prioritize competing responsibilities, solve practical problems, and adapt to changing situations. Computer Skills Working knowledge of Microsoft Office and the ability to learn electronic health record and documentation systems. Certificates, Licenses, Registrations Valid Ohio driver's license and reliable transportation required. Physical Demands Frequent standing, walking, sitting, reaching, and occasional lifting up to 25 pounds. Reasonable accommodations may be made for qualified individuals with disabilities.
