Bilingual Spanish Health Coach & Educator (Home Visits | Maricopa County, AZ | Mileage Reimbursed)

This is a highly field-based position focused on conducting in-home visits with members across the community. The Health Coach & Educator builds trusted relationships with members, caregivers, and families by providing wellness coaching, health education, support, and encouragement to help improve overall health and well-being.

Success in this role requires comfort working independently in the field, managing a daily schedule of member appointments, and traveling throughout an assigned service area. • What to Expect in This Role
• Conduct approximately 6-8 in-home member visits per day
• Spend the majority of your workday in the field meeting members in their homes
• Build trusted relationships with members, caregivers, and families through health coaching and education
• Work independently while managing a daily schedule of member visits
• Daily local travel is an essential component of this role
• Member visits are generally organized by geographic service areas or quadrants to support efficient scheduling and reduce driving time
• Mileage reimbursement is provided from the time you leave home until you return home
• Schedule: Monday - Friday, 8:00 AM - 5:00 PM Mountain Time
