About the role

Serves as a Case Manager for students with IEP or IFSP, acting as a liaison between educators, families, and clinicians. Manages evaluations, annual reviews, and enrollment protocols while ensuring all required documentation is maintained.

Position Type/Expected Hours of Work - Full-Time; Monday – Friday 7

30am-4:00pm, flexible to the needs of the programs.This is a 12-month position.

Qualifications

Bachelor’s degree in a health or human services field preferred. A reasonable combination of education and practical work experience that demonstrates the ability to perform the essential functions of the position will be considered. Valid New York State Driver’s License. Must have excellent writing skills, be an excellent communicator and have a working knowledge of computer programs including but not limited to Word, Excel, PowerPoint. Work Authorization/Security Clearance – Authorized to work in the United States. Summary/Objective - Will serve as the Case Manager for students with an IEP or IFSP. Essential Functions – Reasonable accommodations may be made to enable individuals with disabilities to perform the essential functions: Act as a liaison between educators, schools, families, doctor’s offices, clinicians etc. Responsible for administrative duties/clerical support as assigned. Work in coordination with the EI Director and SPED Coordinator to ensure that all on-going case management is completed as required. Accept, disseminate and track evaluations for EI and Preschool programs. Manage Initial or Reassessment/Supplemental evaluations for EI/CPSE from start to submission. Manage and assist in Annual Review and Enrollment protocols. Coordinate EI 6-month IFSP Reviews. Responsible to send, track and receive prescriptions in a timely manner. Collaborate with the Children’s Services Administrative team to ensure Special Education Students have all required documentation at all times to ensure there is no lapse in service. Complete multiple systematic tasks in IEP Direct/Cleartrack, iCentral, EI Hub and CPSE Portal etc. Manage telephone communications on how to refer children for services through EI or Pre-K. Competencies – Ethical Conduct Organization Skills Detail Oriented Confidentiality Time Management Personal Effectiveness/Credibility Energetic Supervisory Responsibility – None Travel - Travel for home visits and school districts in the tri-county area is required. Work Environment – This position operates out of the Bright Beginnings Early Learning Center and may require lifting of children, playing with students on the floor or outside. This role routinely uses standard office equipment such as computers, phones, photocopiers, filing cabinets and fax machines. Physical Demands - The physical demands described here are representative of those that must be met by an employee to successfully perform the essential functions of this job. While performing the duties of this job, the employee is regularly required to talk or hear. The employee frequently is required to stand; walk; lift; use hands to finger, handle or feel; and reach with hands and arms Work may require lifting, pushing, or pulling in excess of 30 pounds as there may be a need to be lifting children, as well as playing with students on the floor or outside.
