About the role

The Case Manager/Success Coach will support families through Head Start and Community Services programs by providing coaching, resource coordination, and case management. They are responsible for conducting home visits, maintaining accurate records in ChildPlus, and facilitating workshops to help participants achieve personal and professional goals.

UCCA is seeking a motivated professional to support families through Head Start and Community Services programs.

This role combines family coaching, resource coordination, male engagement initiatives, and case management to help participants achieve education, employment, and personal success.

Schedule: Monday–Friday 7:30am-4:30pm (40 hours/week)

Why You’ll Love Working at UCCA Eligible employer for the Public Service Loan Forgiveness (PSLF) Program Medical, Dental, and Vision Insurance through BCBSNC Company Paid Life Insurance and Short-Term Disability Insurance 401(k) (Eligibility is after one year of employment) 12 Paid Holidays Paid Annual & Sick Leave Tuition Assistance opportunities Employee Referral Program Professional development and career growth opportunities Meaningful work serving children and families in our community What You’ll Do Recruit eligible families and program participants Develop and follow-up on Family Partnership Agreements and service plans Connect families with community agencies and support services Provide coaching and mentoring related to education, employment and personal success Determine eligibility and assist participants with service applications Facilitate workshops based on assessed needs and available resources Conduct home visits and follow up on referrals and family needs Maintain accurate records and data entry within ChildPlus Collaborate with site staff and community partners to coordinate services Complete needs assessments and ROMA-based service planning Assist with program recruitment and outreach efforts Support Head Start and CSBG performance goals and standards

Qualifications

Bachelor's Degree in Social Work, Sociology, Psychology, Human Services, or a related field One to three years of related experience working with children, families, or community programs preferred Experience in case management, family services, community outreach, or social services preferred Strong communication, organization, and relationship-building skills CDL required within 90 days of employment Valid driver's license required Bilingual preferred Physical Requirements Must be able to travel within the service area, conduct home visits, attend community events, move throughout program sites, and occasionally lift or move up to 50 pounds.
