About the role

Develop, implement, and evaluate health education programs for diverse community groups and patients. Provide one-on-one counseling and facilitate workshops on various health topics to promote positive behavior change.

Description ESSENTIAL FUNCTIONS

Develop, implement, and evaluate health education programs targeted towards patients, schools, community organizations, and/or selected groups Provide one-on-one health counseling to patients on health topics to promote behavior change Plan, organize, and implement health education workshops and trivia for adults, adolescents, and children.

Topics may include, but are not limited to: depression, heart disease, HIV/AIDS, sexual health, stress, drug and alcohol prevention, diabetes, nutrition, fitness and exercise, self

esteem, safety, asthma, hygiene These workshops may be conducted off-site at schools, shelters or community centers Create and/or enhance existing educational materials as needed Research and collect data for current and future health education programs Act as a resource in health education to our patients, TFH staff, community partners, etc. Be knowledgeable of current issues related to Health Education through professional publications and research Assume responsibility for professional development and job-related competency Attend in-service trainings and meetings as required by TFH; may serve on various committees as needed Perform other duties as assigned The Health Education Specialist will participate as a member of a group of professionals, dedicated to delivering comprehensive, holistic health education services to at-risk children, adolescents, and the community.

Environment

The office is temperature controlled and noise level is kept to a minimum in accordance with professional office/clinic practices. Appropriate workspace will be made available consistent with the position’s job functions. The agency adheres to a no smoking policy.

Demographic, Registration, Appointment/Scheduling, Scanning, Triage, PMH/Vitals, Orders, Immunizations/PPD, Progress Note, Labs, Medical Summary, Telephone Encounters

Requirements

Background and Education: Bachelor’s Degree or higher education, experience conducting health education programming for people of all ages in various settings. Excellent computer skills utilizing Microsoft Office programs and are proficient in research. Have the ability to provide health education presentations to various groups and community organizations. Possess group facilitation and group dynamic skills. Possess excellent written, oral communication, and strong public speaking skills. Bilingual Spanish speaking is required. Possess organizational, project management, and problem-solving skills. Strong reporting and analytical background and the ability to multi-task, while working well in a team-oriented environment.
