Job overview

The Community Health Worker/Parent Educator (CHW/PE) plays a dual role in supporting families with young children to build strong, stable, and nurturing relationships. The CHW/PE provides direct case management and resource navigation for caregivers, facilitates parenting education classes (in-person and virtually), and works collaboratively with the Learning Inspiring Families Together (LIFT) and the Relationships in the Mix (REMix) teams to promote child development, family well-being, and access to community resources. This position requires strong communication skills, cultural humility, and a passion for helping families thrive.

Reports to: Program Manager

Location: 1500 Market Street; some travel required throughout Philadelphia

Schedule: Availability during daytime and some evening and weekend hours required

Responsibilities

  • Provide ongoing parenting skills education groups for couples and co-parents at various community-based sites, using established curricula.
  • Provide individualized case management to enrolled caregivers, including goal setting, regular check-ins (in-person, by phone, or virtually), and referrals to services such as healthcare, housing, mental health, food assistance, and early education.
  • Conduct intake and social determinants of health (SDOH) screenings with new participants; help create and monitor family goal plans.
  • Facilitate or co-facilitate evidence-informed parenting education classes, both in-person and virtually, using the LIFT/ (REMix) curriculum and approved materials.
  • Support caregivers in developing skills in communication, emotional regulation, conflict resolution, positive discipline, and understanding child development.
  • Track participant progress, attendance, and goal achievement; maintain accurate and timely records in the program database (e.g., Apricot).
  • Distribute and collect pre- and post-program surveys, satisfaction surveys, and event feedback.
  • Collaborate with early learning specialists and community partners to connect families with early learning events and resources.
  • Assist with tracking participant progress to ensure course completion.
  • Assist in identifying, recruiting, and engaging clients for the program.
  • Complete program reports and other written documentation as required.
  • Utilize a variety of contact methods to reach participants.
  • Participate in regular supervision, case conferences, trainings, and weekly team meetings.
  • Work with the program team to assist in the delivery of other program activities.

SKILLS

  • Excellent oral communication skills
  • Proficiency with Microsoft Office and web-based data collection applications
  • Ability to engage underserved and hard-to-reach populations
  • Ability to facilitate group classes and work one-on-one with participants
  • Strong organizational skills, including the ability to prioritize multiple assignments
  • Knowledge of issues related to child development, family life skills, child abuse prevention, trauma-informed care, healthy relationships, and healthy parenting
  • Bilingual abilities (English/Spanish or other) preferred but not required
  • Lived experience as a parent or caregiver is highly valued
  • Knowledge of community resources and social service systems in Philadelphia
  • Knowledge of positive parenting principles and practices
  • Comfortable working with adults in low-income communities
  • Strong interpersonal, listening, and communication skills

Education: High school diploma or equivalent required

Experience

  • At least one year of relevant experience in case management, health education, parent education, or community outreach
  • Experience with clients who may struggle with substance abuse, mental health challenges, barriers to employment, and/or have been justice-involved
  • Strong experience with adult education and/or group facilitation
