The Outreach Counselor provides direct, ongoing weekly support to assigned youth and families within the Intensive Home-Based Therapeutic Care program, tailoring services to meet unique needs while maximizing time at home and in the community. Key duties include supporting Individual Treatment Plan goals, assisting in identifying strengths, developing self-management skills, and maintaining professional boundaries to ensure safety.
Candidates must possess a High School Diploma, GED, or equivalent, along with a minimum of two years' experience working with youth and families. A valid driver's license and acceptable driving history are also mandatory requirements for this role.
