Core responsibilities
The transition facilitator implements the clinical case management component of the treatment plan, partnering with social workers and probation officers to develop individualized transitional independent living plans (Tilp) for transitional-Age youth. Responsibilities include assisting youth in developing self-Sufficiency across five life domains, providing life coaching, and maintaining required documentation.
Requirements summary
A bachelor's degree with five years of relevant experience, or an associate's degree with seven years of experience working with severely emotionally disturbed young adults is required, though a bsw or msw is preferred with 1-3 years of human services experience. Bilingual proficiency in spanish is mandatory for this role.
