Care Management Coordinator

The coordinator implements and sustains the evidence-Based healthysteps model, delivering interventions to families with children aged 0-5 who have developmental concerns or social needs, often participating in in-Person primary care well visits. Responsibilities also include maintaining community resource directories, managing a family support line, providing appropriate referrals, and accurately documenting all clinical activities in the electronic health record.

Candidates must possess a master's degree in social work or a related field, along with 1-3 years of relevant experience working with children and families. Successful completion of the healthysteps specialist training is required within six months of hire, and nys licensure must be obtained within six months if applicable.
