Blended Case Manager

The blended case manager supports the physical and behavioral health needs of children by identifying strengths and accessing necessary resources. They also advocate for school placements and collaborate with service providers to ensure families receive appropriate care.

Candidates must hold a bachelor's degree in a social science field or a high school diploma with two years of relevant experience in mental health. A valid driver's license, reliable vehicle, and clean criminal background clearances are required.
