Job description

Care Coordinator – Youth Crisis Outreach Team (YCOT)

Role Responsibilities

  • Successful candidate will perform clinical screenings in schools or home settings for youth ages 3-17 experiencing a Mental Health crisis.
  • Candidate must be highly competent in clinical triage, lethality assessment, de-escalation skills and engagement techniques.
  • Candidate will use due diligence in determining the cause of each crisis and seek collateral information from others involved, i.e. hospital staff, law enforcement, school staff and family members.
  • Candidate then consults with Texana psychiatrist or LPHA to obtain recommendations and helps facilitate that recommendation.
  • Candidate will also coordinate necessary services including crisis follow-up and relapse prevention, referral and linkage to outpatient services.
  • Candidate will continue to work with the youth in crisis for 30-90 days on mental health recovery items and assisting the youth and family on needed resources to enhance the youth’s and family’s well-being.
  • Candidate will work a 40-hour week Monday through Friday with possible exceptions to meet program needs.
  • Candidate will be officed in Rosenberg Texas but will provide community-based work to the client and their family in the home, school or community setting.
  • Flexible hours are required.
  • Candidate reports directly to the Crisis Services Manager and MCOT Supervisor.
  • All services provided must comply with HHSC standards as well as adhering to and promoting Texana’s compliance policy.
  • Candidate is responsible for accurate, timely documentation and data entry.
  • Travel within Fort Bend County is required.
  • Salary
  • 90% $23.02 hour to start.
  • 95% $24.30 hour at 6 mos. with satisfactory performance
  • 100% $25.58 hour at 1 year with satisfactory performance
  • Required Education and/or Certifications, Experience, Skills
  • Bachelor’s or advanced degree from an accredited college or university in counseling, educational psychology, early childhood intervention, gerontology, human growth and development, medicine, nursing, physician assistant, psychology, rehabilitation, social work, sociology or special education.
  • Valid Texas Driver’s License in good standing
  • 2 years working in the Mental Health field with crisis experience. QMHP-CS or LPHA
  • Preferred Education and/or Certifications, Experience, Skills
  • Master’s degree in counseling or human services related field
  • Spanish-speaking
  • 3+ years working in the Mental Health field with crisis experience. QMHP-CS or LPHA
  • Job Related Physical Requirements
  • Must be able to perform SAMA and CPR.
  • Must be able to perform moderate lifting and carrying (15-44 lbs.), pushing, stooping, repeated bending, reaching above shoulder, simple grasping, hearing, walking, sitting and twisting.
  • Must have ability to see, write, operate office equipment, operate a motor vehicle and have depth perception.
