Care Coordinator/Life Skills Instructor- Home & Community Based

The care coordinator/Life skills instructor will use a strengths-Based, recovery-Focused approach to teach adults living with serious mental illness the necessary skills to achieve their individual recovery goals. This role also involves connecting these individuals to their community resources.

A bachelor's degree in social work, psychology, counseling, or a directly-Related human service field is required, along with two years of directly related professional or nonprofessional experience. Candidates must also maintain a valid driver's license and have access to a private car for transportation.
