Care Manager

Care managers provide outreach and enrollment services for children eligible for new york state’s children’s health home program, gathering assessments and creating comprehensive plans of health care. They coordinate services, document progress, and ensure the continuing involvement of families and service providers in executing the plan.

Candidates must possess a bachelor of arts degree along with two years of relevant experience serving children and families in fields like child welfare, developmental disabilities, or social services. Strong written and verbal communication skills are necessary, along with the ability to work effectively across diverse cultural communities.
