Care Manager

Care managers provide outreach and enrollment services to eligible children, gathering assessments and creating comprehensive plans of health care while coordinating necessary supportive services. They are also responsible for documenting all case activity, administering cans-Ny, and convening regular meetings to review progress and confirm eligibility.

Candidates must possess a bachelor's degree along with two years of relevant experience serving children and families in fields such as child welfare, developmental disabilities, or social services. Strong written and verbal communication skills are necessary, along with the ability to work effectively across diverse cultural communities.
