Case Manager, Children's Intake

The case manager is responsible for providing assessment, linking, coordinating, and monitoring services for children and families. This includes developing treatment plans, conducting assessments, and ensuring continuity of care.

Candidates must have a bachelor's degree in social work, psychology, or a related field, along with relevant experience in working with minors and their families. Additionally, they must obtain the children's mental health professional credential within 120 days of hire.
