Youth and Teen Mentor

The teen transition mentor will provide direct support and potential case management duties to individuals on their caseload, while collaborating with the management team to ensure service quality. Responsibilities include supporting youth participants with disabilities during after-School and summer hours to help them develop independence, social, and life skills.

Applicants must be at least 18 years old and possess a valid driver's license with access to reliable transportation, meeting wisconsin's minimum auto insurance requirements. Flexibility to adapt to changing schedules, including occasional weekend availability, is necessary, with bilingual spanish and english speaking being preferred.
