  • Sentara Health Plans is hiring a Prevention Nurse Specialist! This is a remote part-time position within day shift hours. This is not a work from home role and requires onsite participation across the Hampton Roads region.

· Experience in community nursing · Experience in health prevention and/or health education · Experience providing care to patients with diabetes or cardiac risk factors Primary responsibilities include: We are seeking an engaging and mission driven health professional to deliver high-quality community and workplace wellness initiatives. In this role, you will contribute to a variety of health screenings and immunization events designed to empower individuals to take charge of their well-being. You will conduct one-on-one preventive screenings, including blood pressure assessments and fingerstick biometric testing, while using motivational interviewing and patient centered communication to help participants understand their results and take meaningful steps toward reducing cardiovascular and lifestyle related risks. As a trusted health partner, you’ll provide personalized guidance on key wellness topics such as tobacco cessation, adult immunizations, heart health, cancer prevention and early detection, and effective self-care practices. You will also administer seasonal influenza vaccinations in alignment with clinical standards and safety protocols. This position is ideal for someone who thrives on connecting with diverse populations, enjoys delivering supportive health education, and is passionate about improving health outcomes through hands-on engagement.
