Community Health Worker - CPEP & Emergency Department

The community health worker serves as a liaison between the health system and the community, facilitating access to services and improving service delivery. They deliver services to vulnerable individuals, conduct assessments, and provide education on health conditions.

Candidates must have a high school diploma or ged and 1-3 years of community involvement experience. A valid nys drivers license is required, and the ability to earn a chw certificate is preferred.
