Care Coordinator

The care coordinator guides families through enrollment, assessment, and treatment planning for comprehensive behavioral health services at the birth to five center of excellence. This role involves collaborating with providers, facilitating team meetings, ensuring continuity of care, and maintaining medical records to enhance early childhood outcomes.

A bachelor's degree in a related developmental or behavioral health field is required, along with knowledge of child development, disabilities, behavioral health, and mandatory home visiting experience. Candidates must also possess reliable transportation, a valid arizona driver's license, and an arizona fingerprint clearance card.
