- Summary: The Case Manager for THP-NMD works directly with foster care youth (18yrs. – 21yrs.) and foster care young parents to support them in all life domains that prepare them for independence and self-sufficiency. The Case Manager has a small caseload of up to 12 youth who are living in residential homes throughout Santa Clara County. The youth are attending school and/or employment learning life skills such as shopping, meal preparation, maintaining a home, sharing space with others. The youth are focusing on higher education, careers, trade school, well-being, making permanent connections with caring adults, managing their health and dental care, and for some, parenting skills.
- Essential
- Duties/Responsibilities: Complete assessments and intakes on new youth being admitted into THPP. Conduct initial intake process for all new youth admitted into programs. Develop, implement, and continuously monitor individualized Needs and Service Plans (NSP) with the client within 30 days of admission, updating every six months thereafter. Provide intensive life skills coaching in areas including independent living, financial literacy (budgeting, banking, and money management), employment readiness, and vocational development (resume building, job search). Assist parenting young adults with childcare arrangements. Promote and support educational attainment (diploma, college), including monitoring attendance and academic performance, assisting with credit attainment, and attending IEP meetings as applicable. Ensure comprehensive linkages and referrals to necessary community resources (e.g., medical/dental, mental health, substance abuse, legal aid (LACY), reproductive health, and cultural activities) to meet the young adult's full range of needs. Coordinate care and maintain effective communication with key stakeholders, including Placement Workers, family members, ILP Case Managers, and other supportive adults. Participate in all required team meetings (e.g., CFTs, TDMs, MyTime). Lead discharge and aftercare planning from the point of admission, focusing on building long-term, supportive adult connections and ensuring a smooth transition post-program. Ensure strict adherence to Title XXII regulatory compliance for the program and the facility. Maintain a safe and compliant living environment, ensuring the site is in good repair, neat, and tidy, and promptly reporting all maintenance or safety issues to management. Ensure accurate and timely documentation of all client records (assessments, NSPs, contacts, and outcomes) in the electronic health care record (e.g., AWARDs), maintaining compliance with all CCL/Title XXII regulatory standards. Report all incidents verbally and in writing to appropriate parties per Title XXII regulations. Conduct frequent evaluation and assessment of youth's physical, psychological, and emotional needs. Carry on-call phone as part of rotation with other case managers, and respond to after-hours emergencies. Participate in Agency Program Quality Improvement (PQI) and Safety initiatives. Regular attendance and performing other duties as assigned are requirements of the position. Other duties as assigned.
- QualificationsEducation and
- Experience: Bachelor's degree (B.A. B.S.) from four-year College/University or 3 years of equivalent working experience. Knowledge of needs of foster youth, and experience working with culturally diverse youth required. Experience with Community Care Licensing (CCL) programs a huge plus!

- Skills/Abilities: Current CPR and First Aid Certification required (or complete within 30 days of hire). Valid California driver’s license with access to a personal vehicle with current insurance that can be used for company business. Fingerprinting/background check as required by the Department of Justice and/or Title XXII regulations. TB test required.

- Skills: Ability to read and interpret documents such as safety rules, operating and maintenance instructions, and procedure manuals. Ability to write routine reports and correspondence. Must be able to communicate effectively.

- Ability: Ability to solve practical problems and deal with a variety of concrete variables in situations where only limited standardization exists. Ability to interpret a variety of instructions furnished in written, oral, diagram, or schedule form.

- Skills: To perform this job successfully, an individual should have knowledge of Internet software Spreadsheet software and Word Processing software. Gmail and Google Suite experience desired. Internet research skills are a plus.

- Demands: While performing the duties of this job, the employee is regularly required to sit; stand, use hands to finger, handle, or feel; reach with hands and arms and talk or hear. The employee is required to stand and walk. The employee must occasionally lift and/or move up to 20 pounds. Specific vision abilities required by this job include close vision, color vision and ability to adjust focus.

Salary Range
$25.00- $28.00 per hour.
