Health Educator and Navigator

This role involves developing and implementing health education initiatives focused on overcoming barriers to healthy behavior change and facilitating access to social support services for chronic disease management and prevention. The incumbent will also coordinate a navigation system to connect community members with local services addressing social determinants of health.

Candidates must possess at least an associate's degree in a health field or equivalent experience, be fully bilingual (English/Spanish), and have comprehensive knowledge of social determinants of health affecting the latino immigrant community in the dc metro area. Required experience includes two years in community health education/Clinical services and two years with program data reporting.
