School Based Student Assistance Adult Community D/A Case Manager 2

This dual role involves conducting level of care assessments and coordinating treatment referrals for both adolescents in westmoreland county schools and adults in office/Community settings. Responsibilities include linking clients to ancillary services, maintaining referral source relationships, and participating in a 24/7 on-Call rotation.

A master's degree in a related social service field is required, or a master's in an unrelated field with a minimum of two years of related social services experience. Candidates must also pass required criminal and child abuse clearances and be available for after-Hours telephonic on-Call rotation.
