Registered Practical Nurse - Home Peritoneal Dialysis Teach (Talent Pool, 2026)  

The home dialysis registered practical nurse (Rpn) reports to the manager of home dialysis and is responsible for supporting the chronic kidney disease (Ckd) team by educating, encouraging, and training patients and families to independently perform hemodialysis treatments at home. Clinical duties include daily care delivery, training, assessment, support of home hemodialysis patients, collaborative care planning, and dialysis machine management.

Applicants must possess current registration with the college of nurses of ontario and current bcls certification, along with demonstrated clinical expertise in hemodialysis and/Or peritoneal dialysis. A minimum of 5 years of nephrology nursing experience specifically in home dialysis providing both peritoneal dialysis and hemodialysis care is required, along with a valid driving license.
