Reporting to the Manager of Home Dialysis, the Home Dialysis Registered Nurse (RN) is responsible in providing support to the interprofessional Chronic Kidney Disease (CKD) team in readying patients and families to do hemodialysis treatments independently at home. Primary focus will be on providing education, encouragement, and training to patients and families as they embark on doing their own dialysis treatments at home. Clinical responsibilities of this position include but are not limited to daily care delivery, training, assessment, and support of home hemodialysis patients and families; reporting and collaborative care planning with primary nurses and the interprofessional CKD team, and dialysis machine management related to dialysis treatment delivery. Applicants must have experience in Hemodialysis.
