30pm No call, weekends or holidays required! The ideal candidate will have at least 2 years of RN experience with Diabetes exposure. CDCES Preferred!

Role Purpose Provides Diabetes and Nutritional educational services and community resources for inpatients and outpatients. Maintains a case load of patients and promotes program development.

Responsibilities

  • Monitors patients response and modifies plan of care accordingly. Provides accurate documentation as required. Delivers direct patient care by assessing and evaluating patient's physical, psychosocial and emotional needs. Develops and implements individual plans of care in accordance with the established guidelines and standards of nursing care. Educates patients and their families on how to manage their condition.
