SNAP- Case Manager

The stop now & plan (Snap) case manager is responsible for the daily case management of snap participants, including conducting intake assessments, monitoring progress, and facilitating the snap curriculum with fidelity to the model. This role also involves liaising with community agencies, completing required data reporting, and preparing materials for group sessions.

A bachelor's degree in social work, psychology, or a related field is required, along with 3-5 years of human service experience. Preferred experience includes working with structured groups and parent education groups, and desired experience involves community/Agency liaison work.
