Adolescent Mental Health Support & Travel Guide

The guide will establish and maintain trusted relationships with patients and families using a patient-Centered, strengths-Based, and trauma-Responsive approach while co-Developing and supporting adherence to individualized care plans. Responsibilities also include frequent travel to homes, providing coverage for other guides, acting as a conduit to the full care team, and leading care coordination.

Candidates must possess an associate's, bachelor's, or master's degree and have 3-5 years of relevant experience, along with a reliable vehicle, active driver's license, and insurance for in-Home visits. Experience working with serious mental illness or adolescents is a bonus, and applicants must be at least 18 years old.
