Job description

Position Summary

The purpose of the facilitator is to deliver evidence-based curricula to youth in school and community-based sites. The facilitator will implement and deliver programming funded by the Arizona Department of Health Services that provides abstinence and contraception education to youth. In addition, the facilitator is responsible for creating and fostering relationships with new sites and implementing program and curricula with fidelity.

Pay range

$17.78 - $20.19 USD per hour

Job Qualifications

  • Minimum High School diploma or equivalent.
  • 1 year of experience working with youth and conducting public workshops/trainings/classes. (Equivalent combination of education and experience will be considered)
  • Must possess a valid Driver’s License and have reliable transportation.
  • Must possess or have the ability to obtain an (IVP) “Identity Verified Print” Fingerprint Clearance Card and be in good standing with the AZ Department of Public Safety.
  • Bilingual (Spanish/English)

Preferred Qualifications

  • Bachelor’s degree in public health, social work, or related field.
  • Experience delivering teen pregnancy prevention programs
  • 3 years of experience working with youth and conducting public workshops/trainings/classes.
  • Curricula for youth: Wyman Teen Outreach Program, Love Notes Evidence Based Program, Making Proud Choices, Positive Prevention Plus, Be Proud! Be Responsible!, Cuidate, Promoting Health Among Teens
  • Ability to develop and foster positive and professional relationships with youth
  • Knowledge of facilitation and training engagement with youth and adults
  • Possess strong verbal, written and interpersonal communication skills
  • Ability to effectively outreach to parents and community sites to secure implementation sites
  • Ability to effectively prioritize and manage time
  • Knowledge of Microsoft Office Suite (primarily Word, Excel, PowerPoint, SharePoint)

Responsibilities

  • Delivery of the Arizona Department of Health Teen Pregnancy Prevention Program utilizing ADHS TPP approved curricula.
  • Responsible for serving number of participants to ensure program goals are being met as required per our grant.
  • Initiate and contribute to internal collaboration efforts within the Family Services Department.
  • Collaborate with other Arizona Department of Health Services contractors to build relationships and strengthen community network.
  • Provide follow-up contact with participants to ensure participation and attendance.
  • Attend individual and group supervised meetings to discuss programmatic goals, updates, and field challenges.
  • Attend community meetings, events, and workshops for purposes of program outreach and networking.
  • Ensure that program data pertaining to participants and incentives is accurately entered in a timely manner in each respective database platform (i.e., ADHS SharePoint, Wyman Connect, internal data tracking systems).
  • Responsible for maintaining records up to date and provide necessary information needed for the completion and submittal of reports outlined in the Due Dates and Deliverables ADHS document (i.e., monthly, quarterly, annual reports).
  • Secure documents in a locked cabinet to ensure confidentiality.
  • Meet and coordinate with collaborative partners to plan most effective timeline for program delivery.
  • Establish new partnerships and maintain relationships with existing community partners.
  • Comply with mandatory curricula training requirements and participate in additional professional development opportunities (may include outside of Maricopa County).
