Transitional Care Coordinator - Los Angeles

You will be responsible for administrating, developing, and facilitating case management services for assigned residents. You will act as a liaison between residents and various stakeholders to ensure successful transitions back to the community.

Candidates must demonstrate empathy and a genuine desire to work with troubled teens, along with passing background checks and having a valid driver's license. At least one year of experience working with at-Risk juveniles or related fields is required, with a bachelor's degree preferred.
