Caregiver Services Case Manager

The case manager will implement programming directed by the manager, including support groups, educational opportunities, outreach, and information/Referral services for caregivers. Responsibilities also involve maintaining secure client files, attending outreach events, and acting as an agency ambassador in served communities.

A bachelor’s degree in social work is required, along with a minimum of three years' experience with community resources or programming for older adults, and specific experience serving caregivers or relatives raising children. Candidates must also have experience and coursework in gerontology and psychotherapy, and obtain certification in evidence-Based programming within six months.
