Case Manager - Home at Last

The case manager provides specialized support to young adults experiencing homelessness by developing individualized service plans and connecting them to essential resources. They also maintain accurate documentation, facilitate life skills training, and collaborate with community partners to ensure participant success.

Candidates must hold at least a bachelor's degree in a social services field and possess experience working with at-Risk youth or young adults. Strong communication, relationship-Building skills, and the ability to travel to community locations are required.
