Patient Navigator/CHW- Claremont Academy

The community health worker/Patient navigator will outreach, identify, recruit, and support patients to engage in care and care management services. They will also provide patient education and advocacy to assist patients in accessing services.

Candidates must have a high school diploma or ged, with a bachelor's degree preferred, and at least two years of relevant experience. Bilingual skills and a community health worker certification are also required.
